Where we should write the note?

Click on the user's profile. (at the bottom of any of his posts)
On the profile where it says, "Warnings: 0", right below is "View User Notes", click it.
Now, at the bottom is a form where you can add a note.
These notes are just for the staff to view.

Click on "Profile" (bottom of any of user's posts)
Over on the right, it says, "WARNINGS: 0". Right under that is "View user notes"

alternately, you can go through the "Members" page, Search members, then click on the member's name
